Gain XXXX pounds, pay a penalty. Pull out an outfit of hers (no, not a bikini, not lingerie, nothing revealing, just a nice outfit) that she used to look good in, but no longer fits. When she can fit into it again, big reward. If she can fit into it by a certain calendar date, the reward is that much bigger. (The next outfit you try this with can be a step sexier, but wait until she's slimmed into the first one.) On a more subtle level, not part of the written agreement, give compliments when progress happens. I don't suggest you complain when it doesn't, just be silent. People can hear silences, and understand their meaning. There will be setbacks, so don't panic. Christmas to New Years especially. Setbacks are recoverable.

I don't expect you to like this, given that you already feel that you do a lot for Wife and get little in return. I don't promise that Wife won't tell you, with more than a little resentment, that she didn't marry Ivan Pavlov and she's not your dog.

OTOH after 6-7 years you should know what really motivates Wife, and from your post it's clear you haven't tried it. On the carrot end, you've been too stingy, and on the stick end, you've said nothing cautionary for 3 years. Enuf. Be diplomatic but firm. You are prepared to reward her merely for doing what she previously swore she'd do, but you will hold her to her promise. Perhaps she'll listen. Good luck.
