Oh OP, as soon as I saw the age gap and how long you've been together, I knew you were going to have been isolated and he would be jealous and insecure. There's a reason when a grown man or woman dates a teenager who they should have nothing in common with. It's because people their own age are experienced enough not to put up with their bullshit. This relationship feels bad because it is bad. I would plan your exit as if he is going to try and prevent you from leaving, because I really think there is a strong chance he will.
Find yourself somewhere to on-line, gather your documents and important possessions and either move out then tell him via phone, or make sure you have someone with you.
You need to overhaul your diets. Working out does little on its own. Tell her you want to make healthy lifestyle choices. You want to eat better and workout consistently. Give her reasons, feel better, more energy, healthier, etc. Then ask her what she thinks about the two of you doing it together, as a team.
