Embarrassed, why? Do not hide things from these people. They love and care about you. Take note of things you feel like you need to hide, they can offer insights. Are you embarrassed because your boyfriend was incredibly unkind to you in this situation, putting your health (and life) at risk? Probably? If you are embarrassed that this is the person you're dating, make a plan. If you want to stay in the country you are in, people have given you ideas about that. If you want to go home, you need to sort out something while you heal until you can fly.
If you are embarrassed of yourself, don't be. You are scared of hospitals which caused you to delay a bit, but you did listen to your body (eventually) and get yourself the help you needed before it got worse. You did a thing that scared you a lot because you knew you had to. I know that was naked for you. Should you probably have seen a doctor sooner? Yeah. But that's done now, you can't go back and do it differently.
Have you considered peri menopause as part of the issue? She needs to see her GYN. And, marriage counseling.

It sounds like you dodged a bullet if she doesn’t come back. This is an insane level of jealousy, the kind that gets aggressively controlling if you have female coworkers or cousins.
