Honestly, if he did leave you for this it would be better for it to happen now then for him to find out a few years from now when your lives are even more entwined. I get your stress and desire to hide this. You really don't need to approach this as an “I need to cheat” talk. I would pick a time when you're both relaxed, in private and sober then say something along the lines of “Hey just a heads up, ended up hanging out with that guy friend I used to have at the end of my girls night the other day but he got weird so I bailed and blocked him.” You should be able to talk it out from there.
